Swmming Declaration Form

Hi all campers~~

In addition to filling up the indemnity form, there is also a Swimming Declaration Form to be filled as well. An email will be sent out soon to arrange a meeting time and place for collection of the camp fees, NUS acceptance letter, indemnity form, and swimming declaration.
